Surgical Treatment of Congenital Nystagmus with Head Turn


Abstract
Congenital nystagmus with head turn is jerky type nystagmus of which amplitude and frequency often vary in different position of gaze and which is associated with eccentric null point {neutral zone) where there is little or no movement of eyes. This may assume an abnormal head position to obtain best visual acuity. To correct the abnormal head turn, the eves should be moved in the direction of the quick phase of nystagmus or in the same direction as the face turn.
The authors experienced 5 patients with congenital nystagmus and abnormal head turn and chin elevation over 250. On 3 cases among these, we performed "Parks¢¥ modified Kestenbaum procedure", 4 horizontal recti muscle surgery on two cases with abnormal head turn and 4 vertical recti surgery on one case with chin elevation. We also performed "Augmented modified Kestenbaum procedure" on two cases with head turn over 350. In all patients, the abnormal head turn and chin elevation disappeared or improved in severity and visual acuity also improved after surgery.
